"In Gentoo, the "multilib" use flags determines if you wish to compile a 32 bits version of your glibc libraries when you are on a 64 bits environment, which are the core libreries for every linux system. You need that libraries to be able to run any 32 bits software of any kind, it doesn't matter if it is propietary of open source (yes, there are also open source projects that has not been ported completely -or at all, or "it's just impossible"- to 64 bits, for example: wine or grub). It will also allow you to run 32 bits precompiled packages, like mplayer-bin, mozilla-firefox-bin and openoffice-bin, if you don't want to wait for these long compiles.

You need multilib as well to run flash, since it is only distributed in 32 bits form.

Additionally, Gentoo offers some additional support for 32 bits packages, you can find some 32 libs bundled in portage with a package name that always starts with "emul-...". Those libraries are additional libs that are precompiled on 32 bits, and can be used on amd64 systems to run things like opera, some games, or even the ati-drivers (though I think that the ati stuff is changing now)."

* I would go with multilib

The emul- multilib method was removed a week or so ago.

multilib is the default. As KEA0463 says, non-multilib restricts you to pure 64 bit code. Grub will still run at boot time but once the kernel no longer supports 32 bit code, you cannot install grub again.

Keep in mind that you usually choose a multilib on no-multilib stage3. Its possible to convert from a multilib to no-multilib install but not the other way, so make your mind up time is really install time, not desktop profile selection time.
