I have a Dell ultrabook (XPS9350) that has been running Gentoo for a couple years now with no issues. It's worked on AC and on battery, suspend and hibernate worked. All was rosy. Then the kernels went past 4.12.5, and I have had a consistent problem that when the AC cord is unplugged, either the screen freezes, no keyboard input works, but the trackpad still moves the cursor around, or the SSD remounts itself read-only. This has applied to every kernel past 4.12.5. Recently on 4.12.5, I've had to shutdown the machine and start it back up every couple of dozen suspends or it has the same error under battery power. My install is unstable, so I'm used to a certain amount of glitchiness, but this has defeated my ability to figure what the heck is wrong.

I have tried turning off all power management in plasma with no effect. I have tried turning off HD management in laptop-mode-tools, also with no effect.

Unplugging / replugging the power-cable with 4.15.1 gives this in user.log

Please ask for any other information that may help in solving this issue. An ultrabook is not much use if you can't unplug it.

I had the same symptoms in a thinkpad T440S

but in that case it was SATA Link Power Management
switching to "min_power" when unplugging. Forcing
the system to use "medium_power" (via TLP) fixed
it (the TLP setting is: ATA_LINKPWR_ON_BAT=medium_power)

Since you haven't run into this problem before it is
likely something different

I haven't installed TLP, instead I've been relying on laptop-mode-tools. If a solution can't be found, I'll try TLP and see if that sorts the issue, but to be honest, I'd rather not unless it is necessary.
